I'm not sure if that qualifies as a twist as such considering the following:
-John Doe decided not to kill Mills when he had a gun to his head
-John Doe then called Mills to say how much he admired him and envied his life
-John Doe surrendered himself to Mills covered in blood before finishing his masterpiece
-It is stated that there was blood from an unidentified victim on John Doe

I'll admit that I didn't necessarily expect a head in a box, but it was signposted a long way out that Paltrow was the target of his envy and when they were in the car together, John Doe's goading made it fairly obvious that he wanted Mills to become wrath and complete his work.

In any event, se7en is a great film.
