Apparently he was pulled back but at no point does Fry have hold of him. The ref has no idea what's happened and goes with the crowd.

Exactly. The behind the goal angle is the one and it’s similar to the angle the referee had. There is no contact. They are both jostling in a genuine attempt to get the ball. Until Stewart throws himself to the floor outside of the box. It’s a series of errors that ruined the game and cost us.From the video above there is zero contact between their legs though? He booked Howson for nothing other than being a bit late and ignored a few naughty challenges and handballs by the mackems.
No foul but a close contest for a ball that was overrunning towards our keeper, a dive by Stewart into the box and that was all the excuse the ref needed to close the match down.

Shearer was great at it, until Uriah Rennie saw through what he was doing and sent him off, Rennie was soon discarded and the red over turned.90% of time, the doubt goes with the defender, but not on this one, then decided it was in the area which again was debatable then sends Fry off. Ref couldn't have done more to help Sunderland in that sequence of events. Shows how football results hang on such small differences and key decisions. Even after that Sunderland got lucky with the actual penalty itself.
