We are going there for a long weekend in February. Like you we haven't been for a long time, 2015 was the last time we were there I think.
As far as I can remember there isn't a direct metro or train service from the airport to the city centre so it's either the bus (public or Airport Express) or taxi.

As far as things to do is concerned, probably the same as the last time you were there. Charles Bridge, the Castle, Wenceslas Square, Old Town Square, Petrin Hill etc.
We're off to see a gig at the o2 arena which will be our main focus.
The Vytopna railway restaurant/bar is great. It’s in the main square and a miniature railway runs all around it and brings stuff to your table. The castle is worth a trip. You can’t go wrong. We booked a transfer from the airport to our hotel.
We really enjoyed the Národní Muzeum.
The building itself is worth a visit in itself. Impressive architecture inside and out.
There was a WW2 exhibition when we were there last year , which was fascinating.
I worked there in the late 90's. My old boss Gordon, runs a bar James Joyce. It does a good mix of Czech and British food.... I know it's not very cultural but he's a good bloke and will look after you.
This was an art nouveau classic that survived the communist era. It looks from the photos that it has been somewhat cleaned up. Maybe less 'local' than it used to be. It was a bit of an arty haunt.
Get the 22 or 25 tram which runs from one end of the city to the other, it’s quite a good way to see the city.
We stayed at the Lindner Hotel up by the castle which was very nice, and the tram stop was very close. A boat trip on the river is also worth the experience.
