I've had it for two different reasons, first one was because of badly fitting footwear to my flat/ wide feet but second bout I've had for about 6 years now. Second bout was caused by a nerve injury, it's effectively what retired me from football (middle aged 5-A-Side) as the pain was the most around the instep of my strong foot. My other foot was just for standing, so was game over for me, learn to play two footed kids. Had about 10 different types of insoles which made it marginally better, it's never been "fixed" but that's likely due to the nerve taking years to recover. It comes and goes with me though, and is becoming less frequent, so could probably play footy again if legs were not so weak now.
 
Roll your foot over a tennis ball several times a day, sorted mine out in days
Yes they say as you get older that's a good exercise to do to loosen up & stretch the arch tendons before a run or big walk etc.
