Included in the 343 is Tavs spot kick v Norwich, he did officially score but the ref made the wrong decision

Probably not

The 1st retaken penalty that I seen was an FAC 4th round replay v Millwall at AP
Big Johns 1st spot kick was saved but the ref judges Bryan King, I think was his name, moved to soon, the kick was retaken & bingo, in the back of the net

IIRC, Yakubu missed the 1st double retaken penalty at the Rivetside
